AmeriBen Card? Here's How Your Coverage Works in Denton, TX

AmeriBen runs the paperwork for your employer's health plan — but the name that decides where you can be seen is the network printed on your card. AmeriBen plans use several national networks that Evolve Health participates in. Here's how to check yours in two minutes.

Verifying your coverage is easy: bring your insurance card and photo ID, and with your date of birth we can check your plan and confirm you are in-network before your visit — so you can book with confidence.

What AmeriBen is

An administrator — not your network

AmeriBen is a third-party administrator (TPA): a company employers hire to run their self-funded health plans — processing claims, issuing cards and handling the day-to-day administration. AmeriBen is not an insurance network, and clinics don't sign contracts with AmeriBen itself.

In a self-funded plan, your employer pays the medical bills and rents a provider network to decide which clinics and hospitals are covered in-network. That network's name — not the administrator's — is what determines your coverage at any clinic, including ours. Our employer & self-funded plans guide explains how the pieces fit together.

The network on your card

AmeriBen plans use networks we participate in

AmeriBen-administered plans run on a variety of national networks, depending on how each employer sets up its plan. The ones we see most — Cigna, Aetna, MultiPlan / PHCS and First Health — are all networks Evolve Health participates in (see the full list). If one of those names is on your card, there is a good chance your visits with us are covered in-network. Every employer's plan is set up a little differently, though, which is why we verify your exact card before you book.

Find your network

How to find the network name on your card

  • Front of the card: look for a network name or logo — often in a corner, in finer print than the AmeriBen name
  • Back of the card: the claims address and phone numbers usually belong to the administrator; a network logo often appears there too
  • "PPO" wording: if the card mentions a PPO network, that network's name is the one that matters
  • Still not sure? Call us at (940) 257-2002 — describe the card and we will sort it out with you

Questions, answered

AmeriBen FAQs

Is Evolve Health in-network with AmeriBen?

AmeriBen is a plan administrator (a TPA), not a provider network, so clinics do not contract with AmeriBen directly. What decides your coverage is the network named on your insurance card. AmeriBen-administered plans use a variety of national networks — including Cigna, Aetna, MultiPlan/PHCS and First Health — and Evolve Health participates in those networks. Call us at (940) 257-2002 and we will verify your exact plan before you book.

My card says AmeriBen — where do I find my network?

Look for a second name or logo on the card — on the front, often in a corner, and on the back near the claims information. That network name is the one that decides which clinics are in-network for you. AmeriBen's name appears because AmeriBen processes the plan for your employer.

What if my AmeriBen plan uses a network Evolve Health is not in?

You can still be seen as a self-pay patient — sick visits are $75 flat, and we will always tell you the price before you are seen. Call us first and we will help you check every option.

How do I check my coverage before I book?

Call us at (940) 257-2002 with your card handy and we will verify your coverage before your visit. You can also call the member-services number on your card and ask whether Evolve Health Services (NPI 1407631690) is in-network under your plan's network.
