Services & pricing
Service descriptions, prices, and insurance participation are subject to change. Self-pay prices shown ($75 sick visits; $99 DOT physicals with code EVOLVE) are current as of the effective date. Annual physicals are "often $0 with acceptable insurance" — your plan's terms control.
Third-party services
Booking (OptiMantra), payments (InstaMed), maps, and reviews (Google) are provided by third parties under their own terms. We are not responsible for their services or outages.
