New clinic, new providers — we know first visits can feel awkward. Here's exactly what happens at your first visit to Evolve Health, so you can walk in confident.
Before you come
- Book: call (940) 257-2002 or book online — same-day slots available
- Bring: photo ID, insurance card (if you have one), and a list of current medications and allergies
- Arrive about 10 minutes early for paperwork
At your visit
Your provider will start by listening. What's bringing you in, your health history, your concerns. Then an exam, any needed tests, and a clear plan in plain English. No rushed 5-minute visits, no jargon.
After your visit
Prescriptions go to your pharmacy electronically. Lab results come with a real explanation, not just a portal notification. And you'll have a clinic you can call — with your chart, your history, and providers who remember you.
Cost, upfront
Sick visits are $75 self-pay. Annual physicals are often $0 with acceptable insurance. We're in-network with BCBS Texas, UnitedHealthcare, Oscar, Aetna, HealthSelect, TRS-ActiveCare, and Ambetter — and we'll confirm your benefits before you're seen.
